Hepcinat is a new FDA-approved antiviral drug which is used in combination with other drugs for treatment of Hepatitis C Virus (HCV) infection in adults.

Hepcinat is manufactured by Natco and contains Sofosbuvir, the same active ingredient as Sovaldi, developed by Gilead.

Sofosbuvir belongs to a class of medicines called nucleotide polymerase inhibitors.

Sofosbuvir inhibits an enzyme the RNA polymerase enzyme which is essential for viral replication and thus decreases the amount of Hepatitis C Virus in the body thereby resulting in less liver damage and improved liver function.

Treatment of HCV genotypes 1, 2, 3 and 4 is possible with Hepcinat, however it is never used on its own.

Hepcinat is prescribed in combination with other medications such as Ribavirin and Peginterferon Alfa for preventing Hepatitis C virus (HCV) cells from multiplying in your body.

Hepcinat is sometimes used by people who also have HIV, or people who have liver cancer and are going to have a liver transplant, but Hepcinat is not a treatment for HIV or AIDS.

Compared to previous treatments, Hepcinat based regimens provide a higher cure rate, fewer side effects and a two- to four-fold reduced duration of therapy.

Also the cost of Hepcinat is reasonable – generic Sofosbuvir (Hepcinat) is more than 30 times cheaper than brand Sofosbuvir (Sovaldi), so it can be purchased by anyone.

Dosage

The recommended dosage of Hepcinat is one 400mg tablet taken orally, once a day.

Take Hepcinat at the same time each day with or without food.

If vomiting occurs within 2 hours of dosing an additional tablet should be taken.

Hepcinat is not recommended for administration as a monotherapy.

Hepcinat is usually used in combination with Ribavirin (Copegus, Rebetol, Ribasphere, RibaTab) with or without Peginterferon Alfa (Pegasys, PegIntron).

Recommended Regimens and Duration of Therapy:

- Genotype 1 or 4 Chronic Hepatitis C (CHC): Hepcinat, Peginterferon Alfa, and Ribavirin for 12 weeks.

- Genotype 2 CHC: Hepcinat and Ribavirin for 12 weeks.

- Genotype 3 CHC: Hepcinat and Ribavirin for 24 weeks.

- Hepatocellular carcinoma awaiting liver transplantation: Hepcinat and Ribavirin for up to 48 weeks or until liver transplantation (whichever occurs first).

Missing a dose

Hepcinat is used only when needed, therefore missed dosages are not relevant. Do not exceed the doctor's recommended dose.

Overdose

If an overdose occurs and you are not feeling well, you should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

Hepcinat has side effects. The most common are:

  • fatigue
  • headache
  • nausea
  • insomnia
  • anemia
  • itching

Less common side effects during taking Hepcinat:

  • allergic or hypersensitive reactions (difficulty breathing, difficulty swallowing, swelling, chest tightness, skin rashes, and hives)
  • decreased appetite
  • diarrhea
  • irritability
  • muscle pain
  • flu-like symptoms
  • reduced blood cells
  • severe depression
  • increases in bilirubin levels

Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

Contraindications

Do not take Hepcinat if you are allergic to any of Hepcinat’s components.

Do not take Hepcinat if you have severe liver problems (other than Hepatitis C infection), kidney problems or are on dialysis, if you have depression or if you have had a liver transplant.

Hepcinat is not indicated for treatment of HIV or AIDS.

Be careful with Hepcinat if you are taking such medicines as antiarrhythmics (Amiodarone), anticonvulsants (Carbamazepine, Phenytoin, Phenobarbital, Oxcarbazepine), antimycobacterials (Rifabutin, Rifampin, Rifapentine), herbal supplements (St. Johns’ wort) or HIV protease inhibitors (Tipranavir, Ritonavir).

Hepcinat is not recommended for administration as a monotherapy and should be used only in combination with Ribavirin or with Pegylated Interferon and Ribavirin.

Take extreme care to avoid pregnancy. At least 2 effective non-hormonal barrier methods of contraception should be used at all times during treatment and for at least 6 months following the completion of treatment.

Do not take Hepcinat if you're pregnant or a nursing mother.

Hepcinat is not approved for use by children younger than 18 years old.

Try to avoid alcohol while taking Hepcinat.

Taking Hepcinat will not prevent you from passing Hepatitis C to other people. Take necessary precautions as Hepcinat does not prevent you from passing Hepatitis C to other people through sexual contact, the sharing of needles or contact with blood.

While using Hepcinat, you may need frequent blood tests.
